To reduce the number of paper notes submitted to the school, the district has created e-forms that can be completed by parents/guardians. These forms are automatically submitted to the appropriate school office. Please print each form after submission for your records.

Some forms require a doctor’s signature or must be hand-delivered to the school. For these, you must fill them out on your device, then download and print the PDF.
